Foresters Prepared third-party payer now available

You asked for it - now you have it! Prepared will now allow a third party payer at point of sale with the new Pre-Authorized Check Plan (PAC) Authorization Form for Foresters Prepared.

The new PAC form and supporting materials are available for download from Foresters^TM producer website:

• Pre-Authorized Check Plan (PAC) Authorization form 105415 US (01/12)
• Revised Foresters Prepared Producer Guide 503358 US (01/12)

This new form, which can be used for Prepared new business, has built-in payer identification questions. No need for a separate payer ID form!
