We have listened to your feedback and implemented a more streamlined process for obtaining signed illustrations from clients. This enhancement is designed to make the process easier for you and your clients.

New Simplified Application Process: 

1. If you are unable to obtain a signed copy of the illustration reflecting the policy applied for, our new Illustration Certificate form may be used instead. Agents are still encouraged to obtain a signed copy of the illustration reflecting the policy applied for at the time of application if possible.

2. This new Illustration Certificate form must be signed by the Agent and the policy owner and submitted to Phoenix with the application.

3. After receiving the application and form, Phoenix will mail a copy of the illustration that reflects the policy as issued along with the actual policy to the owner. A postage paid envelope with instructions for the owner will be included.

4. The owner is required to sign the illustration provided with the policy and return it to Phoenix.

The new Illustration Certificate form will be available on the PartnerCenter in the applications and forms section beginning on Monday, June 25th.
